As Social Care Practitioners we will:​

  • listen to you and communicate with you in a way that means you are able to take part in conversations about your care and support needs
  • value each person's expertise, views and opinions when discussing your care and support needs and preferences
  • ensure we include carers as much as you want us to​
  • when we have differing views, listen and work together with you to explore all options
  • be transparent about the reasons we may need to say no to ensure that you understand the money you have available to meet your care and support needs. This could be your own money, a health or social care personal budget or a budget managed on your behalf
  • discuss the variety of options available to you within the funding identified through your allocated personal budget
  • work together to agree and coproduce the outcomes you want to achieve through your allocated personal budget
  • work with you in partnership to coproduce your support plan

As commissioners we will:​

  • work together with local people, communities and providers to develop opportunities for good quality care, support and community offers
  • listen and respond to what local people are saying and shape the market for care and support​
  • facilitate people, providers and communities to come together to develop and deliver care and support offers within our available resources
  • be the conduit that channels what local people want into the shape of the care and support market​
  • support communities to create their own solutions which enable people to flourish outside of traditional 'services'

To monitor how we are doing we will:

  • care about the quality of care and support services and will work with people to determine what ‘good’ looks like​
  • use quality checkers to independently review the quality of our services
  • undertake audits of quality assessment and support planning, and use case studies​ to improve our services
  • listen to the views and opinions of local people to understand what we are doing well and how we need to improve and develop
  • create opportunities to bring people together to share their views and experiences
