Hospital Discharge team
Hospital discharge is a challenging yet fulfilling role, working as part of an integrated team with health colleagues, supporting someone’s journey out of the hospital back to their own home. The Hospital integrated discharge team (HID) team makes a real difference in an individual’s journey by assessing a client's strengths and choices using the Care Act framework. We assess clients on the wards and in their own homes to achieve the right support required to ensure their care needs are met. The HID team was in the forefront of developing the Discharge to Assess model of hospital discharge which we introduced in 2016, it remains a dynamic, friendly, and supportive team covering discharges 7 days a week.
Reablement team
The Reablement team is a multi-disciplinary integrated team consisting of occupational therapists, social workers, Social care assistants, and physiotherapists working with clients in their own homes. The Reablement service helps people to retain or regain their skills and confidence so that they can learn to manage again after a period of illness. We use a ‘doing with’ approach with an emphasis on participation and partnership in working towards agreed goals in a holistic, strength-based, and person-centered approach. We work with clients for up to six weeks and we work Monday to Friday.
Rapid Response team
The Rapid Response team benefits from being an integrated health and social care team, comprising of social workers, social care assistants, nurses, occupational therapists, and physios. The team provides rapid support to Bexley residents to remain at home during periods of health or social care crisis. The team operates a 7-day-a-week service with staff working a 5-day week including weekends on a rota basis. The team is currently based at Queen Elizabeth Hospital in Woolwich.
Complex Care team
The Complex Care team is a large team split into three Local Care Networks (LCN). Each LCN supports people who are 18 years and above, requiring long-term social care support. Offering the opportunity to work with diverse cohorts of people. The team has both social workers and social care assistants who work diligently to support Bexley residents.
Care Act Assessments, safeguarding enquiries and Mental Capacity Assessments being the core basis of our work means that social workers are challenged to work to a high standard and will need a clear understanding of the full spectrum of our statutory responsibilities.
The team appreciates the daily demands of the role, this means social workers are supported with regular supervision, reflective practice is the norm, and learning and development are key.
Preparing for Adulthood team
The Preparing for Adulthood team (PFA) works with young people with physical disabilities, learning disabilities, and/or autism aged 14 to 25 years old, and together with their parents/carers to support the transition into adulthood. The PFA team works closely with the Special Educational Needs and Disability (SEND), Children’s Disabilities, and Looked After Children’s teams to ensure a seamless move from children’s services into adult social care services.
Community Mental Health team
Mental health services in Bexley offer a co-located model with a tradition of strong partnership working. There is a new mental health hub that brings together Oxleas service, voluntary partners, Improving Access to Psychological Therapies (IAPT), and social care offering a proactive gateway into secondary services.
Primary Care Plus
Primary Care Plus (PCP) pathway, which is a direct link between primary and secondary care services and focuses on tele-triage, providing direct advice and support to GPs and directing service users to the pathway that will specifically meet their needs. The team is a multi-professional team, they are based at the Erith Centre and work 9am to 5pm Monday to Friday.
ADAPT
The ADAPT Pathway provides focused, therapeutic interventions to adults residing within Bexley who require care and treatment for Anxiety, Depression, Affective disorders, Personality disorders & Trauma. The team is a multi-professional team, they are based at the Erith Centre and work 9am to 5pm Monday to Friday.
Intensive Care Management (ICMP)
ICMP is a multidisciplinary team. They work 9am to 5pm Monday to Friday and are based at the Erith Centre. The team provides a pathway of care and treatment for service users who are diagnosed with schizophrenia or bipolar disorder.
Home Treatment Team
Bexley Home Treatment Team is a multidisciplinary team of professionals and support workers who provide short-term help to people who are in a mental health crisis or who require intensive community support following discharge from the hospital and to prevent hospital admission. The team operates a shift pattern covering the whole week including weekends. They are based at Woodlands Unit at Queen Marys Hospital.
Early Intervention Team
The Bexley Early Intervention Team (EIT) provides a community care approach for people 18 to 65 years old who have experienced their first episode of psychosis. The EIT is based at the Erith Centre and covers the whole borough of Bexley.
The EIT is an exciting and dynamic team. The early intervention team can offer a range of interventions including CBT for psychosis and family work. It is a small team that includes a social worker, occupational therapists, psychologists, and a community psychiatric nurse. These services are available from 9am to 5pm Monday to Friday.
