In Bexley our health and Adult Social Care services are integrated through Bexley Care and the Bexley Well-being Partnership.
We are using Local Care Networks as a way of delivering localised integrated health, social care and community support to meet our residents’ health and care needs, taking a Home First approach.
For example, working within local health and care systems to develop:
more effective discharge pathways from hospital including ‘Discharge to Assess’ both at home and in local nursing homes
enhanced prevention of hospital admission initiatives, which are focused on a Home First approach aiming to reduce admissions to long-term care
improving reablement outcomes by working within Bexley Care to offer a coordinated approach to rehabilitation and reablement and reduce dependency on long term home care support
we are looking for providers to suggest or co-produce innovative models of care for our older residents joining us in working with our health partners, who will wrap support around teams or settings.
For example, our integrated care home support team provides a virtual MDT approach to supporting settings to develop personalised approaches for residents, avoiding unnecessary moves and ambulance conveyances
