What the preparing for adulthood team does

The Bexley Preparing for Adulthood Team works with young people from Year 9 (age 13 to 14) who require support with transitioning into Adult Social Care.

The team work closely with other services. These are listed and linked below:

The service aims to ensure that all young people with SEND transitioning into adulthood are able to do the following: 

Prepare for higher education and/or employment

  • Supported internships, apprenticeships and traineeships
  • Finding a job, and learning how to do a job
  • Understanding welfare benefits

Prepare for independent living

Maintain good health
