What the preparing for adulthood team does
The Bexley Preparing for Adulthood Team works with young people from Year 9 (age 13 to 14) who require support with transitioning into Adult Social Care.
The team work closely with other services. These are listed and linked below:
- Children With Disabilities Team
- Statutory Assessment Service (Education, Health and Care Plan Case Officers)
- Public Health Team
- Bexley Community Health Learning Disability Team
The service aims to ensure that all young people with SEND transitioning into adulthood are able to do the following:
Prepare for higher education and/or employment
Prepare for independent living
Maintain good health
Be part of the community
Please note that the preparing for adulthood service only covers social care.
