Qualifying for help and support

If you qualify for help and support, we will work together to figure out which services would best help you meet your needs and goals.

These services can include both formal and informal support from the community and some might not cost anything. This plan will be your care and support plan.

What is a care and support plan?

A care and support plan is a detailed document that outlines the services you will receive, how your needs will be met and who will provide them.

We will look at your finances to see if you need to pay for your care and support. This depends on how much money you have. We will also suggest where to get advice on finances and make sure you are getting all the benefits you should be.

We will check in regularly to make sure you are getting what you need.

If your situation changes, you can ask for another assessment.
